SOAR is refining and testing a peer-mentoring strategy to strengthen the capacity of health systems and families to support youth as they transition to, and engage in, self-management and adult HIV care and treatment. This approach builds on previous Ministry of Health-approved research at Arthur Davison Children’s Hospital with youth living with HIV and their families. Results offer evidence-based guidance and refined tools for youth living with HIV, their families, and their health care providers to improve the care transition process and related HIV outcomes.
